Pramlintide vs cagrilintide

The first amylin drug and the newest — twenty years apart, what changed?

No head-to-head trial — cross-trial comparison
In one paragraphPramlintide proved the amylin pathway in 2005 with three mealtime injections a day: modest HbA1c gains with insulin, and 6–7 kg of placebo-corrected weight loss over a year in an obesity trial that was never taken to approval. Cagrilintide is the same idea made weekly: 11.8% weight loss alone at 68 weeks in REDEFINE 1, and 20.4% intention-to-treat when paired with semaglutide as CagriSema. Pramlintide is approved in the US only; cagrilintide is not approved anywhere.

Verdict

Cagrilintide for effect size, on published phase 3 data, but unapproved. Pramlintide for the only amylin drug a doctor can actually prescribe — and only in the United States, with insulin, for diabetes.

What pramlintide established

Amylin is co-secreted with insulin and is missing in type 1 diabetes. Pramlintide replaced it, and its approval trials showed what the pathway does: slower gastric emptying, lower post-meal glucose, less appetite, and one to two kilograms less weight than insulin alone. The 2008 obesity trial in 411 people without diabetes went further — three-times-daily dosing produced about 6–7 kg more loss than placebo at twelve months — but the company did not pursue the indication, and pramlintide stayed a niche diabetes add-on.

What cagrilintide changed

Lipidation and albumin binding turned a three-times-daily peptide into a weekly one, and a weekly amylin analogue could be paired with a weekly GLP-1 drug. Alone, cagrilintide produced 11.8% loss at 68 weeks in the REDEFINE 1 arm; with semaglutide, 20.4% on the intention-to-treat analysis and 22.7% while on treatment. The combination has two published phase 3 trials, a diabetes trial and a cardiovascular-outcomes trial running.

The line of descent

Eloralintide (Lilly) and amycretin, now zenagamtide (Novo), are the next steps: a selective amylin agonist alone, and a single molecule that does both jobs. Pramlintide is the proof of concept all of them rest on.

Questions people actually ask

Can pramlintide be used for weight loss?
It produced meaningful loss in one 12-month trial but was never approved for it. It is licensed only with insulin in diabetes, and only in the US.
Is cagrilintide available?
No. It is investigational, alone and in CagriSema. Anything sold under the name is unlicensed.
Why is cagrilintide weekly and pramlintide not?
Cagrilintide carries a fatty-acid chain that binds albumin and slows clearance, the same trick that made semaglutide weekly. Pramlintide is cleared in hours.
