Does Survodutide help with body weight?

B
Grade B — probable, not settled1 phase 3 RCT (n=752, 76 wk) · effect size: moderate
Short answerProbable, not settled. Human data exist (1 phase 3 RCT (n=752, 76 wk)) and mostly agree, showing a moderate effect — but too few, too small, or not yet peer-reviewed for a firm answer. SYNCHRONIZE-2, published in the New England Journal of Medicine on 1 October 2026: −8.2% (3.6 mg) and −9.8% (6.0 mg) versus −3.9% on placebo at 76 weeks, counting all participants whether or not they stayed on treatment; at least 5% loss in 57.6%, 64.5% and 35.1%. HbA1c fell 0.9 and 0.8 points against 0.2, from a baseline of 7.4%. The usual pattern: less weight loss when type 2 diabetes is present.

“The mean weight change at week 76 according to the treatment-regimen estimand (which assessed effects regardless of whether the trial regimen was discontinued or interrupted or whether other antiobesity therapies were used) was -8.2% (95% confidence interval [CI], -9.2 to -7.2) in the 3.6-mg group, -9.8% (95% CI, -10.8 to -8.8) in the 6.0-mg group, and -3.9% (95% CI, -4.9 to -2.9) in the placebo group; weight reduction of at least 5% occurred in 57.6%, 64.5%, and 35.1% of the participants, respectively (P<0.001 for all comparisons with placebo).”

Human trials exist and mostly agree, but there are too few of them, they are too small, or one big one is still reading out. Expect the grade to move.

In context

Survodutide is a once-weekly peptide that activates both the GLP-1 and glucagon receptors. In SYNCHRONIZE-1, a 725-person phase 3 trial published in June 2026, it produced 12–13% weight loss at 76 weeks against 5% on placebo, with gastrointestinal symptoms in up to 90% of participants. In people who also have type 2 diabetes the figure was 8–10% against 4% (SYNCHRONIZE-2, published October 2026). Its distinctive result is in the liver: a phase 2 trial resolved MASH without worsening fibrosis in 83% of patients at the top dose. It is not approved anywhere.

Survodutide is investigational. Legal status differs by country — see the regulation section or the jurisdiction pages.
