Does Petrelintide help with body weight?

C
Grade C — weak human evidence1 phase 2 RCT (ZUPREME-2, n=220, 28 wk), topline only · effect size: moderate
Short answerWeak human evidence. 1 phase 2 RCT (ZUPREME-2, n=220, 28 wk), topline only: enough to be interesting, not enough to count on. Where an effect is seen it is a moderate effect. ZUPREME-2 topline, announced by Zealand Pharma on 7 October 2026 and not yet peer-reviewed: 220 adults in the United States with overweight or obesity and type 2 diabetes (mean BMI 36.3, mean HbA1c 8.0%), on metformin with or without an SGLT2 inhibitor, took one of three weekly doses or placebo. At 28 weeks weight had fallen 7.4–9.2% against 2.0% on placebo, by the efficacy estimand; the company says the treatment-regimen figures were "largely consistent" without giving them. HbA1c fell by up to 0.65 points against a rise of 0.23 on placebo. 1.9% stopped treatment for gut side effects against 1.7% on placebo; nausea and vomiting rates were not reported. A press release from a phase 2 trial, so C.

A few small or poorly-controlled human studies. Enough to be interesting; not enough to count on. Most of what gets called “clinically proven” online lives here.

In context

Petrelintide is a once-weekly amylin receptor agonist developed by Zealand Pharma and partnered with Roche. In ZUPREME-1, a 493-person phase 2 trial published in September 2026, adults with obesity and no diabetes lost 7.9–9.8% of body weight at 28 weeks against 1.7% on placebo, and up to 10.7% by week 42. Nausea affected 20% against 6% on placebo; vomiting and diarrhoea were no more common than on placebo. In people with type 2 diabetes, topline results from the 220-person ZUPREME-2 trial (October 2026, not yet published) gave 7.4–9.2% at 28 weeks against 2.0%. A phase 3 programme of about 7,000 people began in September 2026. It is not approved anywhere.

Petrelintide is investigational (phase 3). Legal status differs by country — see the regulation section or the jurisdiction pages.
