Does Eloralintide help with body weight?

C
Grade C — weak human evidence1 phase 2 RCT (n=263, 48 wk) · effect size: large
Short answerWeak human evidence. 1 phase 2 RCT (n=263, 48 wk): enough to be interesting, not enough to count on. Where an effect is seen it is a large effect. Six dose regimens against placebo in adults with obesity and no diabetes. Loss ranged from 9% at 1 mg to 20% at 9 mg; placebo lost 0.4%. Efficacy estimand, so participants who stopped are not fully counted.

What the studies show

Six dose regimens against placebo in adults with obesity and no diabetes. Loss ranged from 9% at 1 mg to 20% at 9 mg; placebo lost 0.4%. Efficacy estimand, so participants who stopped are not fully counted.

“The mean percent change in bodyweight from baseline after 48 weeks (efficacy estimand) was -9% (1 mg, 95% CI -12·6 to -6·3), -12% (3 mg, -14·9 to -9·8), -18% (6 mg, -20·7 to -14·5), -20% (9 mg, -22·7 to -17·5), -20% (6-9 mg, -22·7 to -17·0), and -16% (3-9 mg, -18·6 to -14·1), compared with -0·4% (-2·2 to 1·4) in the placebo group.”

A few small or poorly-controlled human studies. Enough to be interesting; not enough to count on. Most of what gets called “clinically proven” online lives here.

In context

Eloralintide is a once-weekly amylin receptor agonist from Eli Lilly, tested on its own and, more recently, paired with tirzepatide. In a 263-person phase 2 trial published in The Lancet in 2025, the top doses produced about 20% weight loss at 48 weeks against 0.4% on placebo. Nausea was dose-dependent and fatigue was unusually common. Added to tirzepatide in a phase 2b trial reported by press release in September 2026, it took weight loss in people with type 2 diabetes from 14.8% to 23.3% at 48 weeks. Phase 3 trials are running; it is not approved anywhere.

Eloralintide is investigational (phase 3). Legal status differs by country — see the regulation section or the jurisdiction pages.

Questions

Is Eloralintide proven to help with body weight?
No. The human evidence is weak (1 phase 2 RCT (n=263, 48 wk)); LYO grades it C.
What studies are behind the grade?
Six dose regimens against placebo in adults with obesity and no diabetes. Loss ranged from 9% at 1 mg to 20% at 9 mg; placebo lost 0.4%. Efficacy estimand, so participants who stopped are not fully counted.
Is Eloralintide approved for this?
No. Eloralintide is investigational (phase 3); it is not an approved medicine for any indication in the jurisdictions LYO covers.
