Carnosine

CFood supplementPeptides sold as supplements →L-carnosinebeta-alanyl-L-histidineβ-alanine (precursor)
In one paragraphCarnosine is a dipeptide of beta-alanine and histidine concentrated in muscle and brain, sold as a supplement for glucose control, ageing and exercise. A 2025 meta-analysis of eight randomised trials in 377 people with prediabetes or type 2 diabetes found modest reductions in fasting glucose and HbA1c against placebo, with no effect on weight or insulin resistance. The trials are small and short, and oral carnosine is largely broken down in blood before it reaches tissue. It is a food supplement, not a medicine.

What the human research shows

A consistent small signal from small trials. Grade C: the pooled result is statistically clear, the studies behind it are not big enough to be sure of the size, and the mechanism problem — an enzyme in blood that destroys it — is unresolved.

Each score is confidence × effect size, taken from the best-evidenced benefit in that domain: the grade (A–D) sets the confidence, the measured effect sets the size. Hover a domain to see the outcomes behind it. Adverse effects are excluded and listed separately below.

Registered trials

ClinicalTrials.gov lists 37 interventional studies naming Carnosine as an intervention — 25 completed, 1 recruiting, and 4 with results posted to the registry.

A registration is not a result. Anyone can register a study, and many never report — so this list shows what has been attempted, not what has been shown. The graded evidence above is the answer to whether something works.

What it is

Carnosine is made in muscle from beta-alanine and histidine, where it buffers acid during intense exercise — which is why beta-alanine, its rate-limiting precursor, is a sports supplement. It is also a scavenger of reactive carbonyls and an anti-glycation agent in the laboratory, the basis of its ageing and diabetes claims.

How it is thought to work

Proposed actions are anti-glycation, antioxidant and acid buffering. The complication is serum carnosinase, an enzyme that hydrolyses carnosine in human blood within minutes; how much of a swallowed dose reaches muscle or brain intact is unclear, and the glucose effect may run through the metabolites.

What is still unknown

  • Effect size in a properly powered trial; the eight trials total under 400 people.
  • Whether intact carnosine or its breakdown products are responsible.

Questions people actually ask

Does carnosine lower blood sugar?
Modestly, in eight small trials pooled together — about a third of a standard deviation in HbA1c. Not a treatment for diabetes; possibly a small adjunct, if larger trials agree.
Is carnosine the same as carnitine?
No. Carnitine is an amino-acid derivative involved in fat transport; carnosine is a dipeptide. The names get confused constantly.
Is carnosine a peptide?
Yes — the simplest kind, two amino acids: beta-alanine and histidine.
